Birth and Death Certificates
Certified copies of birth and death certificates are available through the County Registrar up to Six months after the event. For more information please call (541) 523-8211

To order before 6 months:
  • Come to 3330 Pocahontas Rd. in Baker City, weekdays between 8:00 am - 4:00 pm.
  • Please bring a current drivers license or other official picture identification.

To Order after 6 months:

An order form may be picked up at the Baker County Health Department for births or deaths occurring more than 6 months ago, or you may contact the Oregon State Vital Statistics at (971) 673-1190. You may order a vital record by mail, in person or by internet. A non-refundable fee for the record search and a certified copy is $20. Additional copies of the same record are $15 each if ordered with the original request.

Birth Certificates

When ordering birth records, please include with your order the full name of the person on the certificate, date and place of birth, parents' names including the mother's maiden name and you relationship to the person on the the record. By law, only immediate family members or legal representatives may order birth records for another individual.

Death Certificates
When ordering death certificates please include with your order the full name of the person on the record and the date and place of the death. For death record orders, include your relationship to the person on the record, or your reason for needing the record.
