Birth and Death CertificatesCertified copies and abstracts of birth and death certificates are available through the County Registrar up to six months after the event. For more information please call (541) 523-8211. To order records six months after an event, please contact the State of Oregon Vital Statistics at (503) 731-4095. You may order a vital record by mail or in person. The non-refundable fee for the record search and a certified copy is $20. Additional copies of the same record are $15 each, if ordered with the original.All government agencies may order Oregon records if they can provide the record information. All federal, state, and county government agencies must pay the $15 search fee for each record. When ordering birth records please include with your order the full name of the person on the certificate, date and place of birth, parent's names including the mother's maiden name and your relationship to the person on the record. By law, only immediate family members or legal representatives may order birth records for another individual. When ordering death certificates please include with your order the full name of the person on the record and the date and place of the death. For death record orders, include your relationship to the person on the record, or your reason for needing the record. |
